I've seen questions like this before. The key is to think comprehensively about who needs to be in the loop when there are changes to a product release. Shouldn't be too hard to come up with a solid answer.
I think the key is to identify all the relevant parties - the client, the internal team, maybe even regulatory bodies depending on the product. Gotta cover all the bases.
I'm a little unsure about this one. I feel like there could be other people or groups that should be informed, but I'm not totally sure who they would be.
