Option D is the clear choice here. Can't have a 'Contact Us' page that's trying to delete the user instead of letting them cancel. Might as well just change the whole page to 'Delete Yourself' and be done with it.

The 'Delete' button is a clear mistake. The Web Content Manager should definitely discuss this with the Web Developer to get it fixed right away. Can't have a 'Contact Us' page with a 'Delete' button, that's just nonsensical.
