I believe it's important to have the steps to reproduce the failure for easier debugging.

Hmm, I don't know. I'd say the type of defect and the actual and expected results are definitely known at this point. But the lifecycle phase, the actual cause, and the steps to reproduce? Nah, those are probably still missing.

Well, this is a tricky one. I mean, the tester just created the incident report, so how could they possibly know the actual cause of the failure, right? I'm going with C, D, and E.
