I'm a bit confused by this question. I'm not sure why the cause of the risk wouldn't be identified - that seems like a pretty important part of a risk assessment. I guess it could be that the organization has completed the assessments incorrectly, but I'm not totally sure about that. I'll have to think it through a bit more.

I'm leaning towards Iterative. That allows for incremental development and regular check-ins, which should help expose issues and changes as they come up. But I'm not 100% confident in my answer.
