I practiced a question similar to this, and I feel like a Service Level Agreement failure could lead to a change, but it might not be a direct service change itself.

I'm not entirely sure, but I remember something about major incidents being more about incidents rather than changes. So maybe B isn't the right choice?

I think a formal closure of services is definitely a service change, so I would go with A. It seems like it would need to be documented.
