I think option C is the correct answer. All the points are within a standard distance of one another, so the tool just assigns a value of 1 to each one. Simple as that.

Option B is the way to go. If there's only one event to collect, then of course the output value will be 1. Duh!

The Collect Events tool is clearly broken. I'm going with option A - none of the points are co-located. That's the only logical explanation.
