Option D is also a good catch. 'Cancel order' is a more appropriate name for that task, since the order has already been cancelled by that point in the process.

I agree with Adrianna. It's a simple mistake, but it can really mess up the flow of the process. The decision diamond is supposed to be for a different decision point.
