I think it's option A. Non-primary address issues are common.

I think option D makes sense too, especially if the source machine field isn't set up correctly. That could definitely lead to mismatches.

I feel like I've seen something about IPv6 before, but I can't recall if our monitoring tool supports it. Could that be option B?

This kind of question reminds me of a practice one we did about mismatched IPs. I think option C could be a possibility if the SNMP polling has the wrong address.
