I was tempted by D, but you're right - the Risk Register is for risks, not issues. Though I guess the line between the two can get a bit blurry sometimes, eh?
A and C seem the most relevant options to me. The Risk Register should capture potential risks and the project's risk tolerance. D doesn't seem quite right - issues are different from risks.
