Duh, it's to stop the comments from being emailed. Why else would it be called 'Internal Only'? These options are too easy, I'm gonna ace this exam!

The Internal Only checkbox is clearly meant to prevent the comments from being emailed. I don't understand why anyone would think it's to limit the view or hide the comments from everyone except the rep.
