I believe HBA card failure could also be a potential reason for the LUN loss.

I think the main reasons are C) switch port exceptions, D) HBA card failure, and A) RAID LUN resource invalid. The FC SAN environment is pretty complex, so these are the most likely culprits for a LUN loss issue.
