I'm pretty confident I know the answer to this one. Peter is talking about how even the best security technologies can be undermined by the weakest link - the human element. Untrained or careless users can inadvertently let hackers in, no matter how strong the technical defenses are.

Ah, I see what they're getting at here. Okta's authorization servers could be used to secure access to API endpoints owned by a third-party, which would be a good reason to build custom authorization servers in Okta. I'll make sure to consider that in my response.
I'm pretty sure change management policies and procedures are a type of preventative control, since they're designed to prevent issues before they occur.
