I wonder if the administrator tried to 'simpli-fy' the ACL and ended up 'simpli-fying' it right out of existence. Citrix, am I right?

I bet it's option D - the Citrix ADC has been restarted without saving the configurations. That would definitely cause the ACL to disappear.
