I vaguely recall something about "ENDSWITH" being used in different contexts. But it feels like "STARTSWITH" is what we actually need here.

This seems like a pretty straightforward case of cellulitis. I'd go with Vancomycin IV and Piperacillin/Tazobactam to cover both gram-positive and gram-negative organisms.
