This seems straightforward - the patient had polyps removed previously, and is now back for a follow-up colonoscopy with no new polyps found. I think the correct answer is A, Z09 and Z86.010.

I don't think K63.5 is necessary since no new polyps were found, so I would go with Z09, Z86.010.
